Registration

The Practice covers a largely rural area of about 100 square miles, including the town of Frome to the south, and extends over the borders into Banes and Wiltshire. We have three surgeries, the main one sited centrally in Beckington, with one to the north at Freshford and one to the south on the outskirts of Frome.

Serving a list of approximately 9,500, we allow patients to see the doctor of their choice regardless of which doctor is their registered GP. All patients have a right to express a preference for a particular practitioner. For continuity of care we recommend that you see the same doctor, where possible, for ongoing problems as this usually works best for doctor and patient. Please be aware that as we work across three sites we cannot guarantee that you will be able to see a particular doctor at a particular site at a particular day.

How to Register

If you wish to join the practice, please bring either your medical card, completed and signed, to one of the surgeries or alternatively forms are available from Reception. In this practice you may choose to consult any of the doctors and not just the doctor with whom you are registered.

More information can be found and registration forms can be downloaded from the NHS Choices website.

New Patient Checks

Health checks are offered to newly registered patients. This is a free health check for you and an opportunity for us to obtain important background medical history prior to your medical notes arriving. Please book your health check and bring along a small specimen of urine for testing by the nurse.

Change of Personal Details

If you change your name, address or telephone number, please give full details in writing to reception, including your post code. Please remember to tell us if details for other members of your family have also changed. Please also inform the surgery if you change your mobile phone number. If you move outside the practice area, you will be asked to register with another doctor.

Temporary Residents

If you are temporarily living in the area (for up to three months), you can apply to register as a Temporary Resident to access our services. You will need to bring form GMS3 which can be downloaded from the NHS Choices website here.

Overseas Visitors

If you are a visitor to this country and need primary care medical treatment, you can make an appointment at the surgery as a temporary resident. Patients from EU countries are entitled to free consultations, as are patients from non-EU countries which are approved for reciprocal treatment. Prescription charges at the standard NHS rates may apply. Patients from other countries will have to pay a private fee for the GP consultation and any medication that is prescribed. Further information can be obtained from the NHS website here.
